Annual Registration Forms

School Registration
The following forms must be completed by every parent at the beginning of the school year (or as soon as possible after your student(s) starts attending AVA). 
 
Just a reminder that all families are also required to complete the Douglas County School District's Express Check-in. Express Check-in is where you update your household contact information and verify that everything is correct. This typically opens for parents mid-July. 
If your student will be walking/biking to/from school, you must complete the Walker permission Form. Please complete one for each student. (You may need to wait to complete it until you know your students' homeroom teacher)
 
If you plan on picking up your student in carpool, you do need to request a new carpool tag for this school year (every year they are a different color for safety reasons). Please complete one per family.
 
We ask that you read the Parent Student Handbook every year and then agree to its content. Please first review the Handbook and then complete this form (one per family). 
 
If you plan on volunteering at the school during the school year, you will need tocomplete this form. One per volunteer/family.
 
Complete this form if you DO NOT want your household information included in the AVA Directory. Only one form per household is required. 
 
We ask that all parents review the list of websites/apps that have been reviewed for use by students at AVA. If there are sites on this list that you do not want your student to use, please complete this form. 
 
All families are asked to review the current Technology Use Policy. Middle School Students will be sent home with an agreement form that they and their parent will be asked to sign. 
 
Throughout the school year, Aspen View Academy teachers may take students to Zach Parrish Park (adjacent to AVA) for special visits and classroom rewards. Before students are allowed to go to the Park, parents or guardians must first grant permission (since it is "off-campus"). Please complete this form for every student attending AVA. (Please wait to complete this form until you know your homeroom teacher). 
 
The school must have a completed Student Emergency Card on file for each student. Teachers will be sending the form home with your student. You can also print, complete it, and send it in. One form per student. 
 
AVA partners with the SchoolPool program to assist parents that are interested in carpooling, walking, or biking to and from school with other families. If you do not want your information provided to SchoolPool, you must complete this form to Opt-Out.  
 
If your student is not a walker, we ask that every family completes this form. In the event that you are unable to pick up your student(s) after school, your student(s) will be sent to BASE (Before and After School Enrichment) until you arrive. There is a $25 cost to attend BASE as an unscheduled drop in.
